In Ontario, as an international student, you can apply for a health card (OHIP card) as long as you meet the following criteria:
1- You live in Ontario for 153 days or more in a year;
2- The 153 days has to happen within the first 183 days of your stay.
3- Your main residence is in Ontario;
4- You also have to meet at least one of the following conditions:
1) You have applied for permanent residency and have been confirmed that you meet the criteria. Your application has not been rejected;
2) You have a legal and valid work permit and you have held a full-time job in Ontario at least for 6 months, ( In this case, your spouse and children can also apply for OHIP at the same time)

How to apply:
You have to bring the following documents to one of the Service Ontario offices (note that it is not Service Canada), and you need to go in person:

  1. A complete application form
  2. Three pieces of original documents:
  • Proof of OHIP eligibility, such as your work permit and your employment letter (at least 6 months of employment)
  • Proof of residence status, such as Ontario driver license or tax return
  • Proof of your identities, such as your passport or a credit card from a bank
